This paper elaborates health management cloud service model which health management combines with cloud service for the elderly in community. It includes the steps and circulation of health data collection,establishment of electronic health records,assessment and classification,health intervention,health tracking,effect evaluation and so on. It puts forward suggestions and countermeasures of that strengthen support and coordination from government,perfect the mechanism of cloud service management mode and technology construction information sharing so as to perfect the operation mechanism of health management cloud service model for the elderly in community.
