[目的] 探究并评价高血压患者对家庭医生签约服务的利用及效果,从而提高农村基层医疗机构对高血压的管理水平和治疗效果,完善家庭医生签约服务制。[方法] 农村某社区卫生服务中心管理的300例高血压患者作为研究对象,将其分为签约组与未签约组,各150例,比较其血压控制情况、对高血压的知晓情况、自我管理能力及对家庭医生签约服务的满意度评价情况。[结果] 干预后签约家庭医生组的收缩压和舒张压明显低于非签约家庭医生组,差异有统计学意义(P<0.05)。签约组对高血压知识的知晓率及自我管理能力明显提高。[结论] 社区需丰富宣传形式,提高签约率;对签约服务内容及收费标准进行改革,提高利用率;增加家庭医生人数,优化团队组建;患者需重视自身健康管理,理性看待慢性病。

Objective To explore and evaluate the utilization and effect evaluation of family doctors' contract service in hypertensive so as to improve management level and treatment effect of hypertension in rural primary medical institutions,perfect family doctors' contract service system. Methods 300 cases of hypertensive in a rural community health service center were analyzed,and divided them into signing group and non-signing group for 150 cases respectively. The two groups' blood pressure control,knowledge of hypertension,self-management ability and satisfaction degree of family doctors' contract service system were compared. Results After intervention,both systolic and diastolic pressures in signing group were significantly lower than those in non-signing group,and the difference was statistically significance(P<0.05). Awareness rate of hypertension knowledge and self-management ability of signing group were higher than before. Conclusions The form of publicity should be enriched in community to improve signing rate. The content and charge standards should be reformed to improve the utilization rate. The number of family doctors should be increased and personnel structure should be optimized. The patients need to pay attention to their own health management and treat chronic diseases rationally.
